The California Family Health Council, Inc. invites your agency to submit a proposal for the 2009 Educational Partnerships to Increase Chlamydia Screening (EPICS) Program. The purpose of this program is to increase access to chlamydia screening for females 25 years and under as part of a broader family planning program.
Agencies may choose to collaborate with educational partners such as high schools and school-based health centers, community colleges and/or universities.
Agencies may request up to $35,000 for this project. We will expect to fund 8-10 agencies with these funds. Grants will be awarded for a 12 month period (January 1, 2009 - December 31, 2009). The Request for Proposals (Word Doc, 173k) describes the program requirements and the process for preparing and submitting a proposal.
Eligible agencies:
Title X Agencies, IPP Sentinel Sites and Family Planning agencies serving high chlamydia morbidity communities - that are not in the city of San Francisco, or San Francisco County
